Initial assessment individual 50 minutes- $165
Individual 50 minute session-$ 140
Family/couples sessions 50 minutes-$175
Late cancellation/ missed appointments-$50 ​​
Eligibility Requirements:

Clients must complete at least 6 sessions before court-related documentation will be considered.

Clients must be in active care for at least one year before employment paperwork (FMLA, STD, etc.) will be completed.

The provider reserves the right to decline requests if these conditions are not met.
Court-Related Services
-
Court Appearance – $1,000 per day (due the day of court)

Phone Calls for Court – $100 per call (up to 1 hour)

Court Documents or Summaries – $200 per document (per client request)
Employment Documentation

Employment Forms (FMLA, STD, Workplace Accommodations, etc.) – $150

Available only after 1 year of active treatment.
